Summary of incident review experience
At NCN Health we routinely conduct clinical reviews on all patients that fit into the following categories:
- category 1/2 transfers from UCC
- transfers from the Acute ward
- any patient involved in a code (Code Grey/ Code Blue)
- any patient that highlights a concern from Medical and/or nursing staff.
This process identifies if further review is required.
I have been a RCA team member for three reviews. Cases are also discussed at the Clinical Morbidity and Mortality meeting held monthly at NCN Health.
