Below is a list of documents that we require you to upload to support your application. Please have your completed documents ready prior to commencing the application process as the application cannot be saved.
What you need to apply
- Referee and approval form signed by your employer
- Current CV
- Photo (see photo specifications below)
- Certificate of Currency (from your current insurer if applicable)
Please note you will be asked to provide contact details that will be published on the SCV website.
Photo specifications
Your photo will be used as a headshot to be displayed on your profile. Please ensure you use a photo with only you in it and that you are clearly visible. Images must be supplied in JPEG or PNG format and no larger than 500 KB.
We recommend images that are maximum 1035 x700 pixels and minimum 450x450 pixels. To ensure your photo is optimised for our website we recommend using an image that has the same height and width aspect ratio.
Certificate of currency
A Certificate of Currency is a document confirming the currency of your insurance and outlining the conditions of cover. This is relevant for privately practising individuals. You do not require a certificate of currency if your substantial employer supporting your application is a Victorian public health service.
