COPD Acute Care Collaborative
Our goal
To improve the delivery and quality of care for people with COPD in Victoria
Chronic obstructive pulmonary disease (COPD) is a progressive lung disease that can make it hard to breathe. It includes conditions such as emphysema, chronic bronchitis and chronic asthma, affecting about one in 13 Australian adults aged 40 and over.
COPD is a leading cause of avoidable hospital admissions in Australia, particularly among Aboriginal people and those who live in regional and lower socioeconomic areas.
We’re partnering with Lung Foundation Australia to deliver a nurse-led collaborative aimed at improving patient experience, quality of care and health outcomes for people with COPD in Victoria.
Together with participating public hospitals, we’re implementing evidence-based improvements to address gaps in care for patients with COPD. In turn, we aim to reduce the likelihood of readmission to hospital and support people with COPD to stay well and stay home.
This project will run until December 2025.
