Heart Failure

Our goal

To reduce avoidable acute care admissions related to heart failure and cardiovascular disease

Heart failure is a significant burden on the lives of many Victorians who can experience ongoing problems that lead to repeated hospital admissions. This reduces time spent doing the things that matter most to people and can have a negative impact on quality of life.

Heart failure is the leading cause of hospitalisation for people over 65 and 25 per cent of heart failure patients are readmitted to hospital within 30 days of going home.

In this Commonwealth-funded project, we worked with Victorian health services to reduce the rate of unplanned readmission by supporting self-management, implementing evidence-based interventions and making sure the critical transitions of care are right.
