Consumer Forum - Diabetes

Complications from diabetes account for seven per cent of potentially avoidable hospital admissions and have serious health and wellbeing implications for those affected and their families.

We're aiming to reduce diabetes related hospital admissions by five per cent, and readmissions by 30 per cent, by December 2023.

As part of the 100,000 Lives program, we’re partnering with health services and consumers to make improvements in the quality and consistency of care for people living with chronic diseases, including diabetes, across the health system.

To achieve this, we want to meet with consumers, families and carers who have a lived experience of diabetes. We want to hear about your experiences navigating the Victorian health system and ideas to improve care in Victoria.
