The statewide Renal transplant consent to waitlist form is now available in eight frequently spoken languages: Arabic, Cantonese, Dari, Greek, Italian, Mandarin, Turkish and Vietnamese. You can use the form with an interpreter to communicate with patients whose first language is not English. 

The statewide consent form will help your patient better understand the risks associated with kidney transplant. Developed with the renal community, it details other issues you would normally cover during a transplant waitlist discussion.

Please note: The form does not replace having discussions with individual patients.
