Direct Reports
Australian Health Practitioner Regulation Agency (AHPRA) registered clinicians or health services can submit confidential direct reports to the VPCC. These reports provide valuable information that is otherwise unobtainable or not notifiable.

Please note: Reporting to the VPCC is independent of professional notification requirements to AHPRA, Department of Health (DH) or Victorian Audit of Surgical Mortality (VASM).

Referrals from other bodies
Coronial e-Depositions
VPCC directly receives and triages relevant coronial notifications, which are then further investigated by our subcommittees.
Sentinel Events
VPCC reviews perioperative Sentinel Event notifications and directly receives relevant referrals. These are triaged by VPCC and investigated by our subcommittees. To support the monitoring, reviewing and analysing of these perioperative cases, VPCC can collect information from health services or individual practitioners. This reporting operates independently of any notifications made to regulatory bodies relating to perioperative harm or death.
Victorian Audit of Surgical Mortality
Through its Perioperative Management Committee, VPCC discusses and triages mortality reports and reviews conducted by VASM under legislated confidentiality provisions. Further investigation of these referrals may require healthcare organisations to provide case-related data or clinical reviews upon requests. This information is protected under confidentiality provisions.
Other request to VPCC
VPCC may receive requests and act on requests to review cases or circumstances from within the Department of Health. This includes the Minister and Safer Care Victoria.
