Description

The Root Cause Analysis and Action (RCA2) workshop is designed for health service workers and consumer representatives who will lead or participate in an adverse patient safety event review using RCA2 methodology with sentinel events and serious adverse events.

A 9-hour workshop will be conducted over two half days. This workshop will provide a practical application of the adverse event review process, using the root cause analysis (RCA2) methodology, for health service staff and consumer advocates in reviewing serious and sentinel events.

To attend this workshop, you need to complete the Fundamentals of Adverse Patient Safety Event Reviews Workshop (or Incident Review workshop) within the last 3 years.

Who should register?

Health service workers from Victorian health services, and consumer representatives who may participate in reviewing a sentinel or serious adverse event in Victoria.

How to register?

To register for this workshop please complete the Expression of Interest (EOI) form. You will be notified of the outcome of your EOI via email.

As capacity is limited, Safer Care Victoria carefully considers and prioritises participants according to need. We will email you the outcome of your EOI application. 

Participants allocated a place will receive a confirmation email including a resource pack with pre-reading materials and other information prior to the workshop dates.

Workshop details

  • Day 1 | 9am – 1:30 pm, 4.5 hours (online)
  • Day 2 | 9am – 1:30 pm, 4.5 hours (online)

* All registered attendees are required to participate in both days of the workshop

Over two half days, participants will practically apply the learnings of the 8 elements of the adverse event review process and RCA2 analysis to a case study - Anthony’s story.  

Day 1 

Setting up the adverse patient safety event review using RCA2 analysis which includes: 

  • Commencing the review process using Anthony’s story:
    • Set up the review process
    • Form review team and gather what happened
    • Drafting a timeline
  • Commencing data analysis using RCA2 methodology 

Day 2 

Continue analysis of Anthony’s story and the adverse patient safety review:

  • Data analysis using RCA2 methodology continued
  • Develop finding statements 
  • Develop recommendations and an action plan 
  • Monitor recommendations

How much does it cost?

There is no charge for attending the above workshops. However, cancellation with less than three working days’ notice or failure to attend on the day will incur a fee of $385 that will be invoiced to your health service.
