Description

Spread over two half days (after an initial tech check and introductions meeting), this is the first in a series of workshops designed to build your knowledge and skills in reviewing adverse patient safety events. 

This is a pre-requisite to future workshops which focus on review methods - RCA², London Protocol and AcciMap (unless you attended our Incident review workshop in the past three years).

This workshop does not cover a specific review method.

If you have attended our Incident review workshop in the past three years, you won't need to attend this course.

Who should register?

Health service workers and consumer representatives who may participate in reviewing a sentinel or serious adverse event.

As capacity is limited, we will prioritise participants according to need.

Participants allocated a place will receive an email with pre-reading materials and other information prior to the workshop.

How much does it cost?

There is no charge for attending the workshop. However, cancellation with less than three working days’ notice or failure to attend on the day will incur a fee of $385 that will be invoiced to your health service.
