This project aims to promote cross-sector collaboration and provide a forum that is committed to continuous learning by reviewing of local data trends, sharing knowledge of current and new asthma care information, and identifying asthma care system gaps and improvement opportunities that will prompt improvement activities at a local level. 

The North Western Melbourne Primary Health Network (NWMPHN) has been collaborating with Safer Care Victoria to establish and oversee a community of practice that fosters intersectoral collaboration and communication. This community aims to promote relationships, knowledge, trust, and efficiencies across the local asthma care system.

As this framework evolves, it could potentially take on a system stewardship function, allowing local stakeholders invested in asthma care to identify areas that need improvement and drive continuous enhancements in the local community.
