Strengthen Integrated Care Pathways

The aim of the Strengthen Integrated Care Pathways project is to improve service coordination and communication across all participants in the Inner West asthma care system to simplify and improve access to supportive asthma care services.
A robust and integrated system is required to support patients in receiving appropriate, streamlined and linked up care no matter where they are located (particularly between acute tertiary and community care settings).
We have recruited a stellar cohort of collaborative partners consisting of two general practitioner clinics, three community pharmacies, two hospitals, and the Community Asthma Program.
As part of this work, teams have participated in five workshops focused on building capability in quality improvement and developing opportunities to foster partnerships across the health ecosystem amongst a diverse background sharing different perspectives.
With the support of cohealth, we have engaged with over 30 consumers to provide direction for the improvement work, including participants from multi-lingual communities and schools.
