On track to keep people with heart failure safe at home

Our Heart Failure Collaborative is now in Action Period 2 after hosting two Learning Sessions, the most recent in May when 83 clinicians from 19 health services as well as our magnificent lived experience consumers gathered in person at the MCG.
Our lived experience consumers shared their own journeys and played key roles in the plenary sessions and breakout rooms.
Planning, sharing and testing changes, our partners are all very clearly committed to improving care for at least 100,000 Victorians.
During Action Period 2, the team is hosting fortnightly drop-in sessions.
Teams are testing change ideas and collecting data to know if their changes are leading to improvement.
The project team is starting to see some shifts in the data. One-on-one team coaching is also occurring.
The project team is also planning regional site visits.
They also issue a monthly newsletter to project participants via Team Assurance.
The third Learning Session is scheduled for late August.
