It has been a big couple of months for the Heart Failure Collaborative! The team hit the road and visited sites across Victoria for one-on-one coaching and witnessed the impact the collaborative is having firsthand.
It was wonderful to reach out and meet some of the project teams and our lived experience experts, gaining insight into the innovative ways they are impacting the health landscape. Heartbeat Ballarat's walking group are playing an active role in not only keeping their community hit and healthy but also playing a vital peer support role for people diagnosed with Heart Failure and other heart conditions. Western Health and Bendigo are also actively embedding lived experience experts into their Cardiac wards, ensuring patients are at the centre of their care and providing much needed peer support at the time of diagnosis.
The Heart Failure Collaborative is now in Action Period 3 after hosting a remarkably successful Learning Session in October, with 85 clinicians from 19 health services taking part in the two-day event.
The project team is excited to be seeing a positive shift towards reaching our goal of reducing 30-day admissions for heart failure clients.
The project team is delighted to announce an extension to the Heart Failure Collaborative so we can continue supporting Victorian health services until the end of June 2023!
