Frailty recognition and response in the community

Summary

It is estimated that more than a quarter of older adults are frail, making them more likely to have poor health outcomes. With an ageing population, itimely recognition of frailty followed by an evidence-based intervention can improve health outcomes. 

We partnered with five health services to introduce an approach to frailty screening and management - recognising and providing timely intervention to patients presenting with frailty and pre-frailty.

Philip Sabien

Philip has held senior executive positions in the food manufacturing sector and economic development and tourism with experience at state and local governments. He moved into the health six years ago.

Philip has food science and human nutrition undergraduate qualifications, a business management MBA and a professional certificate in health systems management. He is also a graduate of the Australian Institute of Company Directors.

Anne Hughes

Anne is a Registered Nurse and completed a master’s in health service management in 2015.

She started her redesign career supporting the implementation of a hospital-wide electronic patient journey board in the Northern Territory. Anne has since gained more than four years’ experience in redesign positions across a several health services leading initiatives to improve patient care in emergency and acute care settings. She is passionate about mentoring and coaching individual improvement leads and teams to safeguard consumer health experiences.

Naomi Winter

Naomi has been a project officer implementing change for more than 20 years but only recently began her improvement and innovation journey. While her focus has always been on building relationships with internal and external stakeholders to improve the delivery of healthcare and effective change management, the difference over the past five years has been the application of improvement science
